Volunteers Registration

I certify that the information on this form is complete and accurate. I understand that the Muslim Society of Guelph will not be responsible for any injury caused during the volunteer service. I will only agree to volunteer for the programs that I am capable to manage and do. If a student, I am volunteering with the consent of my parents/guardians.

I agree to commit on the tasks assigned to me and if I am unable to do, I will inform the Team lead I am volunteering for.

I will be tracking all volunteer hours on the volunteer tracking sheet provided by Muslim Society of Guelph.

I have read, understood and will abide by the rules and regulations of the Muslim Society of Guelph.
